Pentagon doctor claims prescribing transgender drugs to minors can cause psychotic illness to 'melt away'

'I think that more likely it's that when [transgender] treatments are optimized, these diagnoses start to melt away over time,' a doctor treating adolescents at Travis Force Air Base said.
Children taking anti-psychotic medication in the military health system for serious mental illness were later administered "gender-affirming pharmaceuticals," according to Department of Defense health records, with one of its air base physicians claiming the gender drugs could "melt away" psychotic conditions such as a schizophrenia diagnosis.
A study published in The Journal of Sexual Medicine in 2021 entitled, "Mental Healthcare Utilization of Transgender Youth Before and After Affirming Treatment," discussed internal DoD health records from 2010-2018.
The review showed that transgender kids in the DoD health system were being given heavy psychotropic medications such as anti-psychotics while being sent through a "gender-affirming" pipeline.
Drugs given to transgender youth typically include puberty blockers and hormones.
"Among 963 [transgender] youth using gender-affirming pharmaceuticals, mental healthcare did not significantly change and psychotropic medications increased following gender-affirming pharmaceutical initiation," the study said.
"The most pronounced increases in psychotropic medication were in… anti-psychotics and lithium," a mood stabilizing medication commonly used to treat bipolar disorder.
One of the authors, Dr. David Klein, works at the David Grant Medical Center at Travis Air Force Base where he treats military-connected children with gender dysphoria.
In response to the findings of the study, Klein said more surgeries and drugs could change mental health outcomes in the long term.
"[I]t doesn’t mean that the surgery that urologists are considering or subsequent medications… is not going to have long-term benefit. I think that’s an area that we’re still looking at very carefully," he told Urology Times.
Dr. Klein also said that children's psychosis and other diagnoses could "melt away" when transgender medical interventions were "optimized."
"We see higher rates of psychotic conditions and other conditions that might jump out. I think people might be like, ‘Perhaps transgender people are inherently at higher risk for schizophrenia.’ But by looking at these data, in reality the [psychotropic] medications prescribed sometimes may indicate that one's mental health is suffering," Dr. Klein said. "And I think that from a clinician's perspective, or somebody that treats a lot of folks in this scenario, I think these medications and these diagnoses happen when there's not enough care from the start."
"I think this by looking at the rates, you know, psychotic disorders, it's easy to make assumptions [that] this is something inherent to transgender and gender diverse people. I don't think that we know that. And I think that more likely it's that when treatments are optimized, these diagnoses start to melt away over time," he said.
"So more research over time will show that definitively, hopefully."
The Pentagon responded to Dr. Klein's point of view stating, "Hormone therapy is not a treatment for psychotic conditions."
The Pentagon did not directly respond to Fox News' question on whether the military health system continued to provide children experiencing psychotic illnesses with transgender pharmaceuticals.
"DoD clinicians follow current evidence-based clinical practice guidelines for the management of gender dysphoria, published by the Endocrine Society and the World Professional Association for Transgender Health," a spokesperson for DoD, Cmdr. Nicole Schwegman, said. Informed consent is obtained from parents for these treatments, she added.
Dr. Klein, as the program director of family medicine residency, also trains doctors at the California military base. He did not respond to a request for comment.
Another study of military health records conducted by Klein, of which children were 90% of the sample, found that nearly 30% stopped gender-related hormone treatment they obtained from the military health system. The specific reasons for stopping gender treatments were not elucidated in the study.
"Future research is needed to determine how… changes in gender identity, resolution of gender dysphoria, difficulty accessing hormones, or a preference for treatment outside of the [military health system] influence the decision to stop obtaining gender-affirming hormones from the MHS," the study said.
Dr. Stanley Goldfarb of Do No Harm, an organization seeking to restore the medical profession to the Hippocratic Oath, told Fox News Digital that the medical literature does not support minors being given hormones and puberty blockers as a standard of practice.
"The problem is what's going to happen 15 years later, how many of these girls that go through this are going to be miserable and how many are going to be satisfied? I don't know," he said.
Dr. Goldfarb added that the proclaimed need for transgender medical interventions for minors has evolved into a "mythology" and is not in line with what the science currently shows.
"The problem is we have all these questions and what's going on in… the European countries [are saying]… 'We really don't know what's going on here. The evidence that it's beneficial is very poor. We don't think that we should put kids into treatments that are experimental, but we should do experiments to see whether the treatment is beneficial,'" he said.
"It should not be sort of what's been described as the Wild West of America," Dr. Goldfarb continued. "Kids [in America] show up at a clinic, and they say, 'Yep, you're trans. We'll start you on the puberty blockers. Then we'll start you on the hormones.'"
"I don't think there can be informed consent of a child right now. They can give consent. But do they really know what they're saying, particularly about something like this?" he added. "To tell a child, well, you may have bone loss when you're an adult, and you might have osteoporosis, they don't know what that means. They don't know what the consequences of that are."
Klein, as well as other DoD physicians, previously argued in the March edition of the American Journal of Public Health that the only pathway for children of military members who present with gender dysphoria symptoms is to immediately move towards "gender-affirming health care, such as puberty suppression and affirming hormones."
He said that on the basis of "human rights," "youths… have an inherent ability and right to consent to gender-affirming therapy."
He claimed 7-year-olds can make their own medical decisions.
Klein proceeded to blast clinicians who pause before changing a minor's gender to see if they would grow out of the dysphoria, calling it "unethical."
A general issue with studies on transgender medical interventions for minors is the high levels of people dropping out, said Dr. Goldfarb.
"These high dropout rates and there's not adequate follow up. So you really don't know about the people who leave, right? Whether they detransition, whether they got their medication someplace else and are still going through a transition, nobody really knows."

Boston Children's Hospital director calls for drastic increase in capacity for gender surgeries for minors
The director at Boston Children's Hospital said medical school students should learn transgender surgeries in residency programs.
Boston Children's Hospital co-director at its Center for Gender Surgery called for a drastic increase in capacity for what he called "gender-affirming care" (GAC), including surgeries, for kids as GOP states enact bans on the practices.
Oren Ganor is a plastic and reconstructive surgeon who specializes in gender-affirming surgeries. He has previously stated in an email that the hospital is "slightly flexible" when it comes to the age of transgender girls seeking genital surgery," according to a local outlet. The policy had not been finalized, he said, "because of the issue around consent for sterilization."
The children's hospital clarified to Fox News Digital that it does not provide genital surgeries on kids. A patient needs to live in their new gender for 12 months before the genital surgeries are considered.
In a March 14 article he co-wrote with a medical school student, Shawheen J. Rezei, in The Journal of the American Medical Association, he said that the capacity for gender surgeries for kids needed to be increased.
"Physicians who provide GAC will face a greater burden due to constraints in certain states… work to criminalize GAC for adolescents," the article stated. "Especially as certain states work to criminalize GAC for adolescents, there will be an increased flux of patients traveling to seek care in states with more open legislation."
It called for more clinics to be opened up with targeted knowledge on the gender-affirming model. They outlined several steps "to improve capacity for this patient population" since "there will be an increased flux of patients traveling to seek care in states with more open legislation."
The authors recommended medical school residency programs expose future physicians to GAC by "increase training efforts" for "gender-affirming surgeries."
"[P]lastic surgery residencies currently do not have a recommended number of gender-affirming surgery cases that plastic surgery trainees see during training," the authors wrote.
A number of Republican-led states have passed legislation to ban gender-affirming surgeries for kids which they defended in an amicus brief in June 2021, stating that "gender-affirming care" was "experimental."
"We are filing this brief because… we are concerned about the surge in recent years of children suffering from gender dysphoria and other forms of gender-related psychological distress," said Alabama Attorney General Steve Marshall in a statement. "[W]e are concerned because these vulnerable children are suffering greatly and need help."
In contrast, proponents of "gender-affirming care" believe it is life-saving treatment for children with a disproportionately high rate of suicide.
"It is important to recognize that for many gender-diverse young people, the decision to engage in gender-affirming care can be lifesaving and life-changing. Without this care, gender-diverse youth can experience severe distress, anxiety, and depression," the Children's hospital told Fox News Digital. "[A]ny decisions about gender-affirming care should be made after a rigorous evaluation process that involves a psychologist or social worker, a patient's doctor and their parents or guardians."
In August, Boston Children's Hospital posted a video, which was later deleted, where a psychologist explained that ‘a good portion’ of children she sees at the hospital's Gender MultiSpecialty Service (GeMS) clinic know their gender identity ‘from the womb.’"
Boston Children’s Hospital also previously faced a wave of backlash over its Center for Gender Surgery that performs mastectomies on teenagers as young as 15, as well as since-deleted wording on the hospital's website that claimed teens as young as 17 can get vaginoplasties.
The hospital’s website stated as recently as Aug. 12 that "to qualify for gender affirmation at Boston Children's Hospital, you must be at least 18 years old for phalloplasty or metoidioplasty and at least 17 years old for vaginoplasty," according to The Internet Archive's Wayback Machine.
That wording has since been changed to read, "To qualify for gender affirmation at Boston Children's Hospital, you must be at least 18 years old for phalloplasty or metoidioplasty and for vaginoplasty."
A hospital spokesperson said "for surgical consultation, you must be 17 years of age and between 18 and 35 years of age at the time of surgery. We have since updated this to reflect the protocols for the practice, which we have always adhered to (no surgery under 18 years of age)."
After the controversy, the hospital received numerous threats, including some bomb threats.
Massachusets Attorney General Racheal Rollins, said, "I want to say generally that health care providers who support and offer care to gender-diverse and transgender individuals and their families deserve to do so without fear."

Hospital insider warns trans kids on hormones frequenting ER for harming themselves in cry for help

The Maine hospital insider calls chemical sex changes for kids 'cruel'.
An insider at MaineHealth, one of the largest systems in the state, who is familiar with patient care information – including adolescents – is speaking out about transgender children who are taking hormonal pharmaceuticals for chemical sex changes and subsequently showing up with serious mental health crises, including psychotic breaks from reality, in acute emergency settings.
The source made their concerns known to Do No Harm, an organization seeking to restore the medical field to science-based care.
Patients with psych emergencies are admitted to the emergency room in severe scenarios when urgent stabilization is needed. Psych-related emergency room visits are due to the patient being a danger to themselves or others, verbal threats of violence, hallucinations, cutting themselves, and attempting suicide.
"I feel like it's kind of like [the kids'] way of screaming out, ‘Hey, look at me, I need help.’ And however, whatever issue was going on, it kind of shows itself. They could also have things like cutting behaviors or any form of like self-harm. Cutting is one of the more frequently seen ones," the insider said.
As part of the "affirming" model, patients with gender dysphoria are treated in gender clinics where they take cross-sex hormones and puberty blockers. Those drugs are considered by proponents of "affirming care" as "life-saving." Transgender patients have a high rate of suicidality, which supporters of sex changes attribute to lack of family and societal acceptance.
The insider reported that some of those patients who are taking hormones experience serious psychiatric emergencies, including cutting and breaks with reality. The insider also said they were aware that children, mostly girls, 13 and over – who have supportive and "affirming" families – are showing up in emergency settings.
"Parents are encouraging this by talking to staff and saying, ‘You need to call her this boy name. This is how she identifies.’ Like telling staff that we should be doing this. Not the kid."
However, they believe the "affirming model" is "cruel" and questioned why it made sense that patients taking cocktails of psychotropic medications to manage severe mental health issues would be given hormones and puberty blockers, which also have known side effects of depression and anxiety.
"There's so many other side effects… Why would you add that to somebody who's already struggling? It makes no sense to me. I don't even you don't even have to have a degree to figure that out," the insider said.
The affirming model is considered a standard of care in the U.S., despite countries in Europe such as Sweden, the United Kingdom and Finland distancing themselves from its use in minors.
Puberty blockers used on children as young as nine, which are also used to chemically castrate cancer patients, have been linked to sexual dysfunction, a condition that can cause brain swelling, and issues with bone mineral density. Long-term use has also been linked to osteoporosis.
In other cases, practitioners have drastically reversed course after sounding the alarm on the use of the medicines – such as puberty blockers.
Notably, Dr. Marci Bowers, who has performed thousands of genital surgeries, once warned that blockers can cause permanent sexual dysfunction.
"Every single child who was or adolescent who was truly blocked at [early puberty stages] has never experienced orgasm. I mean, it's really about zero," said Dr. Bowers in 2021.

Could it be that because we are still a society repressed to some extent that most people with "issues" keep it closeted but later on, because Western societies have become open and liberal to some extent that, "queer" people start "coming out" and then embrace their true self which consequently destroys their families?
Perhaps this is a vicious cycle we cannot escape from because when we are children we have so many doubts that we dare not come out - not to mention that in our younger days we are not really sure about our true characteristics, and therefore are confused, and chose to remain repressed and closeted. But then, when we are adults, we become confident to come out, and this is where everything goes "wrong" for their families.
It's a tragedy that cannot be avoided, I guess.
